Dr Suhail Alam - Medical Director

Blepharoplasty, also known as eyelid surgery or eyelid reshaping, is a surgical procedure designed to remove excess skin, fat or muscle from the upper or lower eyelids. It helps correct drooping or hooded eyelids, reduces puffiness or under-eye bags, and can restore a more refreshed, youthful, and alert appearance. In some cases, it also improves peripheral vision that is obstructed by excess skin.

As we age, the skin around the eyes loses elasticity, fat pads shift, and muscles may weaken, leading to drooping lids, under-eye bags, and a tired or aged appearance. People opt for blepharoplasty to rejuvenate the eye area, restore facial symmetry, enhance self-confidence, and, in cases where excess upper eyelid skin blocks vision, to improve sight.

For upper eyelid surgery, the surgeon makes a small incision along the natural crease of the eyelid, removes the excess skin and fat, and closes the incision with fine sutures. Lower eyelid surgery may involve an incision just below the lash line or inside the eyelid to remove or reposition fat and excess skin. The goal is to achieve a natural-looking eyelid contour while minimising visible scarring.

The duration of blepharoplasty depends on whether upper, lower, or both eyelids are treated. Surgery on a single pair of eyelids usually takes about 45 to 90 minutes. Procedures involving all four eyelids may take up to two hours.

Scarring is usually minimal as incisions are placed in natural eyelid creases for the upper lids or just below the lash line or inside the lower lids. Scars tend to fade over time and are generally well concealed, leaving a natural and rejuvenated appearance.

Although swelling and bruising diminish within the first two weeks, the eyelids may continue to settle over several months. Final results, including eyelid contour, symmetry, and appearance, typically become apparent around three to six months post-surgery.

Ideal Candidate

Who Can Benefit from Upper Eyelid Surgery?

Our surgeons provide detailed assessments to determine if upper eyelid surgery is right for you. Each consultation evaluates the eyelid skin, muscle, and overall eye health to recommend a personalised approach that enhances both appearance and function.

Drooping or Hooded Upper Eyelids

Patients with sagging upper eyelids may notice a tired or aged appearance, or in some cases, obstruction of peripheral vision. Surgery removes excess skin and tightens the eyelid crease, restoring a refreshed and natural look.

Excess Skin Affecting Comfort or Vision

Upper eyelid skin that hangs over the eyelashes can cause discomfort or make the eyes feel heavy. Candidates with this concern can benefit from surgery that improves both eyelid function and aesthetic appearance.

Healthy Adults with Realistic Expectations

Ideal candidates are generally in good health, do not have uncontrolled medical conditions that could affect healing, and understand the realistic outcomes of upper eyelid surgery. The procedure is tailored to each patient’s anatomy and goals.

Frequently Asked Questions

Results are long-lasting, with most patients enjoying the benefits for five to ten years or more. Natural ageing continues, so some minor changes in eyelid appearance may occur over time, but many people remain satisfied with their results for years.

Preparation may include stopping smoking several weeks before the operation, avoiding certain medications such as blood-thinners, and following the surgeon’s preoperative instructions. Patients are also advised to arrange transportation home and support during the first few days after surgery as vision may be temporarily blurred and swelling present.

Post-operative care involves using prescribed eye drops or ointments, applying cold compresses to reduce swelling, keeping the head elevated while resting, avoiding strenuous activity, refraining from using makeup or contact lenses for a period, and protecting the eyes from sun and wind. Follow-up appointments are scheduled to monitor healing and ensure optimal recovery.

While rare, revision blepharoplasty can be performed if the results are unsatisfactory or if complications arise. According to clinical data, the need for revision occurs in fewer than four in 100 patients. Surgeons will evaluate and discuss the best approach for any adjustments to achieve the desired outcome.

Yes, in cases where excess upper eyelid skin droops enough to obstruct peripheral vision, removing the skin can improve the visual field. This functional benefit is often considered alongside cosmetic improvements.
